Instructions:
MCF will provide guidance to applicable grant recipients on the completion of this form.
For awards greater than $1,000, this is an agreement between the City of Martinez, Martinez Community Foundation, and:
Agreement date
The services rendered pursuant to this agreement will be for the following period:
MCF, on behalf of the City, has determined that it is in the interest of the City to make a grant for such purpose, provided certain conditions are met to ensure the services will benefit the residents of City. The amount of the grant
AGREEMENT
The City, MCF, and the Organization agree as follows:
1. Recitals
The foregoing recitals are true and correct and are part of this agreement.
2. City Grant
The City shall provide the amount listed below (A) to be used by the Organization for (project or program) support as described in the "
Community Groups Funding Program Application" form
. The grant shall be paid upon invoice to the City.
3. Records
The Organization shall maintain records for program review, evaluation, and/or other purposes and make them available to City and/or MCF upon request.
4. Periodic Reports
Upon request of the City and/or MCF, the Organization shall provide reports describing the progress made by the Organization, accomplishing the goals and objectives outlined in the work plan. See After-Action Funding Report.
